Tear Duct Problems

Another area of expertise for Dr. Durboraw is in the evaluation and surgical correction of both acquired and congenital tear duct obstructions.

Commonly Performed Surgeries
Blepharoplasty (Eye Lift)

This surgery addresses excess eyelid skin. If your eyelid skin problem is extensive it may be obstructing your peripheral vision and surgical correction may be covered by your insurance company. This surgery is performed routinely as a cosmetic procedure if it is not extensive enough to be obstructing your vision. You can make an appointment for an eyelid screening

Ptosis Repair

Ptosis is a condition where the eyelid margin has descended and is obscuring the pupil of your eye and your peripheral vision. People often describe their eyes as looking "half mast" when they are trying to fully open their eyes. Sometimes this is combined with excess eyelid skin which can be addressed simultaneously. If your peripheral vision is significantly obstructed, your insurance company will pay for the surgical repair of this condition. Make an appointment for evaluation.

Lower Eyelid Blepharoplasty

Aging of the lower lids can often make patients feel they appear tired or "drawn". Usually the problem involves a combination of excess eyelid skin and bulging fat pockets of the lower lid which accentuate the tear trough and exacerbate the appearance of dark circles. Although this is not considered medically necessary and will not be covered by your insurance, these problems can often be corrected through surgery. Make an appointment for evaluation.

Ectropion

Ectropion is a pulled down appearance of the eyelids and is sometimes a complication seen with aging or with eyelid trauma. Patients may experience exposure related symptoms of irritation, tearing and dryness. Surgery for this condition is considered medically necessary and covered by your insurance.

Entropion

The lower eyelid can sometime begin to roll in with age causing the eyelashes to rub against and abrade the cornea of the eye. The result is significant eye pain, irritation and redness. Surgery to correct entropion is considered medically necessary and covered by insurance.

Tear Duct Obstruction

Tearing and recurrent eyelid infections are often the first signs of a tear duct obstruction. Sometimes a cause can be identified, such as chronic sinusitis, allergic rhinitis or a history of facial trauma such as a broken nose. Most of the time however, we are unable to identify why the tear duct became obstructed. The good news is that surgery is usually very successful at resolving a tear duct obstruction. Surgery Information

In general, most ophthalmic plastic surgery procedures such as the surgeries listed above are performed on an outpatient basis under IV sedation. Fortunately recovery from these procedures is usually minimally uncomfortable but does usually involve some bruising and swelling around the eye area which results in some downtime. Typically patients feel that they show some signs that they have had recent surgery for 1-3 weeks following the procedure. Fortunately, complications such as infection, bleeding and scar formation are very uncommon with ophthalmic plastic surgery – but as with any surgical intervention complications can occur. It is important to notify Dr. Durboraw promptly if you feel that something isn't right after surgery. Most problems are minor and easy to fix.

During your consultation Dr. Durboraw will review specific instructions and recommendations for the type of surgery you are going to have performed. In general, however:

Preoperatively

Stop any aspirin containing products two weeks prior to surgery.

If you are on prescription blood thinners, obtain instructions from Dr. Durboraw regarding the discontinuation of these medications.

Start oral Arnica Montana one week prior to surgery and continue for one week post-operatively. This herbal supplement is available at health food stores such as Wealth of Health in Idaho Falls. (Name should be link to their website). Taking Arnica Montana will reduce the degree of bruising and swelling and make the bruising resolve faster.

Post-operatively

Apply ice to the operated areas for two days following surgery. On the day of surgery, ice application should be almost continuous (with small breaks here and there). You do not need apply the ice while you are sleeping overnight.

Sleep on your back with your head slightly elevated to reduce swelling.

Post-operative pain for most procedures should be minimal and of short duration. Dull headache type discomfort should be the maximum pain you experience for most procedures. Dr. Durboraw will notify you preoperatively if you should expect more pain than this. If you are experiencing more pain than anticipated notify Dr. Durboraw immediately so she can identify and treat the source of your discomfort.

Notify Dr. Durboraw if you are having any excessive swelling or worsening swelling after the first 48 hours. Mild itching is a normal part of the healing process but severe itching combined with increasing redness could be a sign of allergy to the topical antibiotic ointment. Please discontinue the ointment and notify Dr. Durboraw.

Notify Dr. Durboraw immediately if you have any signs of infection such as increased swelling after the first 48 hours, significant discomfort or pressure, discharge or fever.

Do not wear makeup on operated areas for 10 days. Concealer on bruises which are not along incision sites are okay at any time following surgery.
